As the Chief of Maternal and Fetal Medicine, I am committed to serving the patients, trainees, and community of the University of New Mexico through my work in clinical care, education, research, and community service. With the support of our department and hospital we are building a network of high-risk pregnancy outreach clinics and strong tertiary clinical perinatology and imaging resources in Albuquerque to better serve the women of New Mexico. We continue to develop our robust training programs in Obstetrics and Gynecology and Maternal and Fetal Medicine for trainees at all levels. In research we are building on our strengths in clinical care and consultative ultrasound as well as in translational work in novel approaches to fetal assessment, pathophysiology and therapy of preeclampsia, placental aging and telomere pathology, and substance abuse. Finally, our division seeks to leverage our community resources and support to develop a strong statewide regional perinatal care network that will optimize pregnancy outcomes for our diverse population.

Research
research overview
- As Division Chief, I have implemented my philosophy that all faculty members should be engaged in research to some degree. Each faculty member has now developed a research area and multiple studies are now ongoing in new research areas for the division including intrauterine growth restriction (IUGR), ultrasound, perinatal mental health, premature rupture of the amniotic membranes (PROM), placental aging, and anti-angiogenic biomarkers in preeclampsia. I successfully competed for a pilot grant from the UNM Clinical and Translational Sciences Center to support my preeclampsia research work as well as securing funding from the departmental Seligman fund to study biomarkers in severe preeclampsia. I have mentored MFM fellows Nathan Blue in the IUGR work and Vivek Katukuri in the anti-angiogenesis and PROM studies, and am working with MFM fellow Jose Perez on additional studies in perinatal ultrasound. In 2018, my division presented nine abstracts at the Society for Maternal and Fetal Medicine including a plenary oral presentation by MFM Fellow Nathan Blue and two abstracts at the American College of Obstetricians and Gynecologist annual meeting, and has also published multiple papers including three publications to date in 2018 from my IUGR project with Dr. Blue. My senior fellow, Vivek Katukuri, has been invited to give a plenary presentation on his basic science work in cellular mechanisms of fetal brain injury during hypoxia and inflammation at the 2019 Society for Maternal and Fetal Medicine meeting. Recently my division member Danielle Esters, MD, was awarded a pilot grant to begin our joint studies in placental and neonatal leukocyte telomere length and telomerase activity in women who abuse opiates. Our next steps in expanding the research capabilities and activities in the division are to implement the research funding component of the new Luis B. Curet Professorship endowment to support research in our division and to initiate applications to external funding sources including direct NIH support. - Fetal Growth Curves in New Mexico: Effect of ethnicity Retrospective study of approximately 65,000 ultrasound exams to characterize the unique growth curves at high altitude in Albuquerque, New Mexico, with subanalyses to examine ethnicity specific growth curves in Native American, African American, Hispanic, Asian, and Non-Hispanic Caucasian populations.
- Longitudinal Observational Study of Anti-angiogenic Factors in Preeclampsia Observational studies correlating clinical measures of disease activity to maternal serum anti-angiogenic factors. Funded by University of New Mexico Clinical and Translational Sciences Center and Department of Obstetrics and Gynecology, University of New Mexico. Logistical arrangements for patient recruitment, longitudinal follow-up, and laboratory analyses are now complete and patient recruitment is ongoing.
- Ultrasound Predictors of Small for Gestational Age Infants Examination of the ability of multiple ultrasound parameters, timing, and combination of morphometric factors to predict small for gestational age infants. Multiple subanalyses examining timing of ultrasounds, value of multiple ultrasounds, and various computational methodologies to predict SGA at birth. Study supported with local resources and data analysis completed. Six abstracts are accepted for presentation at the Society for Maternal and Fetal Medicine 2018 annual meeting and two abstracts at the American College of Obstetricians and Gynecologists 2018 annual meeting, and publications are being written and submitted. One paper is now published, two are in press and a fourth is in preparation.
Teaching

- Medical Students: I give lectures and informal tutorial sessions during the third year clinical rotation; supervise the participation and clinical education of the students in Labor and Delivery, on teaching rounds, and in the clinic. I provide clinical instruction and supervision to fourth medical students on the clinical service and in the clinic. Interns, Residents, and Fellows: I direct and manage all administrative and educational aspects of Maternal and Fetal Medicine Division including fellow recruitment, curriculum development and implementation, creation of objectives and evaluation processes and documents, management and implementation of 360 degree and portfolio evaluation processes, development of new fellow rotations and learning opportunities, and fellow discipline. As MFM fellowship program director, I am responsible for all fellow educational activities and perform program and fellow evaluations, reports to ACGME, ABOG, and UNM, and site survey preparation. I direct fellow management committees in curriculum, evaluation, and research. I mentor fellows in clinical performance, academic issues, team building, and career development. I give formal lectures to residents and fellows on selected topics. I attend, prepare, supervise, and coordinate protected-time teaching activities. I provide supervision, clinical instruction, and consultation to interns, residents, and fellows on the clinical service, in the operating room, and in the clinic; assist and supervise trainees in surgical procedures in the operating room, labor room, wards, and clinic. I train residents, fellows, and students in subspecialty management in maternal and fetal medicine and in high risk pregnancy procedures such as genetic amniocentesis and targeted ultrasound. I provide feedback to trainees on clinical performance and medical management. I supervise and coordinate all fellow’s research projects. Faculty: I am responsible for faculty recruitment, retention, evaluation, promotion, and discipline for the division of Maternal and Fetal Medicine. I facilitate faculty development in clinical, administrative, teaching, and research arenas. I encourage and mentor junior faculty and assist them in the promotion and tenure process. I develop collaborative relationships between faculty and outside entities, other divisions and departments, with trainees and other faculty, and with each other.

outreach overview
- In 2016 I assumed the position of Chief of Maternal and Fetal Medicine at the University of New Mexico. At that time the division was experiencing a severe shortage of faculty, with only two full-time faculty members, and was unable to adequately cover clinical, educational, or research responsibilities. I immediately set out to recruit faculty and within a year built the division back to five full time and four part time faculty including a double-boarded MFM and Medical Geneticist. I reorganized the high-risk pregnancy clinic into a high-quality service with a focus on customer service, including standardized consultation notes and reorganized scheduling to increase continuity of care with the faculty providers. I found numerous issues with erroneous ultrasound billing and was able to bring the CPT coding to national standards. This brought greatly increased revenue by properly classifying the anatomy survey patients by risk status and eliminating over- and under- coding. Working with hospital partners, I developed a new reporting mechanism in the electronic medical record that pushed out ultrasound reports to the clinicians’ inboxes and brought the ultrasound reading turnaround time down to the American Institute of Ultrasound in Medicine standard of one business day. My work in optimizing ultrasound scheduling has resulted in substantial increases in sonographer daily productivity. With the opening of the Eubank clinic I greatly expanded the availability of MFM and ultrasound appointments, increasing from 5 to 10 ultrasound rooms, doubling the number of MFM clinic visits per week, and partnering with clinic administration to recruit additional nursing leadership, social work, and case management services. I worked on maintaining and developing the perinatal outreach program including strategic planning for permanent sites at three remote locations and recruiting a dedicated nurse coordinator to manage the complex multidisciplinary outpatient antepartum patients. In addition, I collaborated with hospital and medical school business operations on developing appropriate metrics for faculty productivity to inform strategic planning for future expansion of the MFM division as we work on our next step of reaching out to the Albuquerque community referral base. In the area of quality and safety, I took the lead in developing a multidisciplinary departmental formal peer review committee for obstetrical care that reviews cases to inform physician discipline and credentialing as well as identifying systems issues.
